Registration & Forms Spring 2023

Eastern Crew operates as a club, which means in addition to the FinalForms requirements, it would be greatly appreciated if you could register your athlete separately with Forest Hills Eastern Crew Club and pay those dues to the appropriate party. Some athletes will be required to complete the US Rowing online waiver in order to participate in certain Regattas. Coach Norrix will notify those athletes closer to the time.

1) Register with FINALFORMS

Forest Hills Eastern High School Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students online. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice!  You will review your information once per school year to verify information is current and sign the forms.

Enrollment in FinalForms is mandatory for all students planning to participate in sports at Forest Hills Eastern. To register your athlete, please click the link below:

IMPORTANT NOTE ABOUT MEDICAL CLEARANCE: New or Returning to Crew? Please make sure you have a current health form on file with the FHE Athletic department. A CURRENT-YEAR PHYSICAL IS ONE GIVEN ON OR AFTER APRIL 15 OF THE PREVIOUS SCHOOL YEAR.

2) Register with CREW CLUB

New to Crew? 
Please read the Crew Handbook prior to submitting paperwork.

Crew Handbook

  • Please complete the Online FHE Crew Registration, or print this form out and complete by hand.
    • Rower Code of Conduct Contract
    • Travel/Medical Permission Form
  • CREW Dues Payment
    • $425.00 for Spring 2023 (additional rowers in the family: $200 each)
    • Pay with Check payable to FHE Crew Boosters and turn all forms and payment in to: Ruth Brinks(see below)
  • NAME YARD SIGNS (optional): Yard signs with your rower’s name on it are placed along the carpool line during each week we have a regatta. Yard signs are $25 each and are reusable. They can be purchased through the online registration form, in the final section linking you to the CheddarUp payment.

Turn your dues payment and paperwork (if you didn’t do the online registration)in to: Ruth Brinks, Crew Board Treasurer at 750 Purple Creek Ct. SE, Ada, MI 49301.

3) In addition to the above forms, certain regattas will require rowers to sign the US Rowing ONLINE WAIVER in order to participate. Coach will send out notification prior to the events.

  • The US Rowing waiver must be completed each calendar year to compete in any regatta that follows U.S. Rowing rules. All regattas we attend follow this rule.
  • Complete the online US Rowing Waiver with your student (waivers must be submitted by the athlete, parent, or the legal guardian if the athlete is under 18 years old). 
  • Click on US Rowing Waiver – for both novice & returning rowers
    • Under INDIVIDUALS  click “Join” if you are a novice.
    • If you are a returning rower click “Log In” to sign the waiver.(You may have received an email reminder with a link and prompts to complete this)
    • Our club code is: M4C7G
    • If you have forgotten your user name or password there are prompts to help you.
    • There is a $25 admin. fee to be paid to US Rowing ($5 discount if renewing).
    • First time registrants will type in “Forest Hills Eastern” under “team or club name”.

MARK YOUR CALENDAR for these early dates:

  • MANDATORY SWIM TEST on March 16th at the FH Aquatic Center, at 6:45pm.
  • TBD – First day of practice on the water.
  • Calendar for remaining dates (subject to change)

All rowers must complete the FHE Crew Packet & forms and pay fees for both Club and Pay-to-Play. All forms must be in by the first practice or you will NOT be able to participate that day. Late registrations accepted, with Coach approval.
